SBISD requires all students to have met all immunization requirements before admittance to school.

Please provide the school with working phone numbers so that you can be contacted in case of emergency.

Parents will be notified in the event their child's oral temperature reads 100.4° or higher

Keep children home until they are fever free WITHOUT Tylenol for 24 hours.

Please free free to visit the clinic anytime to discuss your child's health needs.

      

 After graduating from the University of Texas Health Science Center at Houston School of Nursing, I worked in the field of intevsive care nursing and then specialized in cardiology. Since entering the field of school nursing in 2003, I have had the opportunity to serve children through the delivery of preventative and acute health care, teaching health education, and promoting a healthy school environment. I have enjoyed becoming a school nurse and believe that healthy students can enjoy higher educational potential.

 

 

      

All prescription and non-prescription medications must be kept in the school clinic and must be brought to the school nurse BY A PARENT when the student arrives at school.

Prescription medicine may only be given when a signed request with a complete dosage instructions has been received from a physician or dentist licensed to practice in the U.S. and the parent/guardian.

Prescription medicine must be provided by the paren't/guardian in the original container, labeled with complete dosage instructions from the pharmacy or physician.

Non-prescription medicine may be given when a signed request with dosage instructions has been received from the parent/guardian.

Non-prescription medicine must be provided by the parent/guardian in the original container with label of dosage instructions.

****Medication forms must be renewed at the beginning of each school year.****

 

Servicios de Salud

Todos los medicamentos con o sin receta deben ser conservados en la clinica y deben ser registrados con el personal de los servicios de salud, cuando el estudiante llegue al esuela. **El padre o guardian necesita traer el medicinas a la enfermera**

Los medicamentos prescritos solo pueden ser dados cuando el medico o dentista autorizados para ejercer en los Estados Unidos y el padre o guardian legal proporcionen una declaracion escrita y firmada con las instrucciones de dosis, pidiendo que la escuela suministre el medicamento. El medicamento dibe ser proporcionado por el padre o guardian y debe estar en su envase original dibidamente etiquetado con las instrucciones completas de dosificacion de una farmacia o medico reconocidos.

Los medicamentos sin receta pueden ser administrados cuando una peticion por escrito, firmada, con las instrucciones de dosifacacion hasido recibida del padre o guardian. Los medicamentos sin receta deben ser proporcionados por el padre o guardian y deben estar en su encase original debidamente etquetado con las instrucciones completas de dosifiacacion.

****Todas las formas para dar medicina aqui en la escuela tienen que estar renovadas el comienzo de cada ano escolar****
